Submit a case

Referring physicians from UC academic health centers may submit active patient cases in advance of MTB meetings. To submit a case, please:

1. Download and complete a case summary form.

2. Upload the completed case summary form and relevant genomics report to the UCCC MTB Secure Box Folder.
